| Our payment policy requires that fees be paid at the time of service unless other specific financial arrangements are made. |

| We accept cash, checks and credit cards. |

|

| If you have dental insurance, please give your completed insurance forms to our administrative staff, and we will be happy to process them for you. |

|
|
| If an involved course of treatment is necessary, our administrative staff will work out a payment schedule with you. Insurance benefits can be assigned toward our payment arrangement. |

|
| Monthly budget payments via credit cards or automatic deductions from your checking account are available. |
